EX-15: the NIE application form, line by line
One page, few fields — and still the most refused step of the whole application. Here is every field, the fee form that accompanies it, and the mistakes the police desk rejects on sight.

Generate the form on the Sede Electrónica (código 012), select “asignación de NIE”, pay €9.84 at any bank or online. The stamped receipt travels with your EX-15 — pay after downloading the 790 so the reference matches.
The 4 refusals we see weekly
1. Wrong 790 box
Ticking “prórroga” or a TIE box instead of NIE assignment. The bank clerk won't catch it — the police desk will.
2. Missing motive
An empty “motivo” field is a same-day refusal. One clear line is enough.
3. Name mismatch
Second surnames, accents, transliterations — the EX-15 must mirror your passport exactly.
4. Unsigned copies
Both copies signed, same pen, same date.
